SUBMISSIONS SHOULD BE EMAILED TO ART@PLANAHEALTH.ORG BY 5 PM on September 1, 2024. Please include the image, your name, and the location photographed. You may submit up to 5 photographs. No images will be entered in the contest if the submitted has not also filled out the below form.

The winner of the first place prize will receive a $250 gift card, second place $150, and third place $100.

Photographs must be at minimum 2980x2384 or taken on an 8 megapixel camera (most cellphones made in the last 3 years qualify).
