Plan A’s Healthy Families, Healthy Futures focuses on improving the health and wellness of parents, caregivers, and families in the Mississippi Delta. Services include free healthcare, care coordination, education classes, support groups, and mommy and me classes.

Our goal is to ensure that all of the families in our community have easy and free access to the healthcare and resources they need to live healthy lives.

Families with infants participating in our classes and care coordination are given Delta Baby Boxes with age appropriate supplies at each session along with a months supply of diapers. Pregnant people participating will receive monthly Delta Mom Boxes with supplies for a healthy pregnancy. Healthcare includes coordination with your existing providers, mobile clinic visits, remote home monitoring of conditions such as high blood pressure, low birth weight, and more.

Throughout the summer we are holding focus groups to learn from our community what their priorities and needs are. Your feedback shapes our program.

Healthy Futures programming is available to parents, caregivers, and infants from pregnancy through infant age 18 months in Humphreys, Bolivar, Washington, Leflore, Sunflower, and Holmes counties.
